Awards will be made at the PVA Healthcare Summit + Expo Renaissance Orlando at SeaWorld Hotel in Orlando, Florida: August 15 - 17, 2019

EXCELLENCE AWARD
Paralyzed Veterans of America will recognize CLINICIANS of EXCELLENCE at the Healthcare Summit + Expo in August.  Nominated by peers, the awards provide national recognition in the provision of exceptional professional achievement, exemplary treatment, strong leadership and personal advocacy to improve the lives of individuals living with spinal cord injury or disease (SCI/D), amyotrophic lateral sclerosis (ALS), or multiple sclerosis (MS).

The award recipients will receive a complete conference package, registration, hotel and travel, to attend the 2019 Healthcare Summit + Expo.

AWARD CATEGORIES

• Physician, Physician Assistant, Nurse Practitioner
• Nurses (Staff Inpatient and Outpatient RNs and LPNs)
• Nursing Leadership (Nurse Managers, Assistant Nurse Managers, Nurse Educators, Wound Care Nurses)
• Social Worker
• Psychologist
• Therapist (Physical, Occupational, Kinesiology, or Recreational)

NOMINATIONS CRITERIA

• The nominee has demonstrated leadership and expertise and is a recognized authority in the area of SCI/D, ALS, or MS clinical service or research.
• The nominee has demonstrated exceptional clinical, scientific, educational, and/or administrative achievements in the care of individuals living with SCI/D, ALS, or MS.
• The accomplishments of the nominee have a positive impact on individuals with SCI/D, ALS, or MS and their quality of life.
• The achievements of the nominee are recognized by peers, professionals, or the community.

To nominate a peer or colleague, submit a one page (500 word) letter of nomination describing the individual’s accomplishments in the criteria above. The nominee’s name, credentials, title / position, facility and contact information (telephone number, mailing and email addresses) should be at the top of the nomination. The nominator’s name, credentials, title / position, facility and contact information (telephone number, mailing and email addresses) should be at the bottom of the page. Letters of support for the nomination are welcome.

ALL NOMINATIONS ARE DUE BY FRIDAY, April 26, 2019 AT 11:59 P.M. EDT

SELECTION
All nominations will be reviewed by a selection committee.  Winners and nominators will be notified by May 28, 2019.
