EXCELLENCE AWARD

The Paralyzed Veterans of America (PVA) Clinical Excellence Awards recognizes and honors peer-nominated caregivers who embody the best values of clinical excellence in the care of Veterans with Spinal Cord Injury or Disease (SCI/D), Amyotrophic Lateral Sclerosis (ALS), or Multiple Sclerosis (MS). 

• Submit your written nomination by June 5^th, 2022
• Winners will receive their award at this year’s PVA Healthcare Summit and Expo

Award winners will receive an all-expenses-paid conference package (registration, hotel and travel expense) to attend the 2022 PVA Healthcare Summit + Expo, at the Omni Dallas Hotel, where they will be formally presented with their award.

AWARD CATEGORIES

• Medicine (Physician, Physician Assistant, Nurse Practitioner)
• Nursing (RNs and LPNs in Inpatient, Outpatient and Specialty Services)
• Nursing Leadership (Nurse Managers, Asst. Nurse Managers, Nurse Educators)
• Social Work
• Psychology
• Therapy (Physical, Occupational, Kinesiology, or Recreational)

NOMINATIONS CRITERIA

• The Nominee has demonstrated leadership, expertise, collaboration and success in measurably advancing care delivery and promoting change for Veterans with SCI/D, ALS, MS, through clinical practice or scientific achievement.
• The Nominee has demonstrated outstanding professional achievement by contributing above and beyond in providing evidence-based practice education to other SCI/D, ALS, or MS professionals, the community and/or Veterans with SCI/D, ALS, or MS.
• The Nominee has measurably improved quality patient outcomes and patient safety as a result of his/her practice in direct or indirect patient care of Veterans with SCI/D, ALS, or MS.
• The Nominee’s accomplishments have enacted national change to improve care delivery to individuals with SCI/D, ALS, or MS.
• The Nominee is recognized as a role model for their exceptional care delivery to patients and their families, delivery of unique wellness initiatives, professional achievement, and dedicated advocacy to improve the lives of veterans with SCI/D, ALS, and MS and their support systems.

NOMINATION INSTRUCTIONS

Please follow all instructions carefully, or your nomination may be considered ineligible.

• NOMINATIONS ARE DUE BY June 5, 2022 AT 11:59 P.M. EDT
• First, list the nominee’s name, credentials, title, facility and contact information (telephone number, mailing and email addresses). 
• Next, write a narrative, no more than 500 words, describing the individual’s accomplishments and reason for your nomination, reflected in the criteria above. 
• Last, list the nominator’s name, credentials, title, facility and contact information (telephone number, mailing and email addresses). 
• Additional letters supporting your nomination are welcome.

SELECTION AND QUESTIONS

All nominations will be reviewed by a multi-disciplinary healthcare selection committee. Award winners and their nominators will be notified by June 30, 2022.  All questions concerning the awards should be sent to summit@pva.org.
